It is in very good condition, no chips, no cracks and no scratches that I can see. I don't know the age or maker. It is an auction item. It does have age glaze crazing so I believe it is old. This measures 4.5 inches high and 4.75 by3.25 inches across. Besides the crazing this piece does have 4 hairline cracks around the edge and some discoloration around the back as well. This would still work great for a dry arrangement or accent piece.
